If you're deep into Diablo 4's late-game grind, Abhorrent Hearts are probably the thing slowing you down most. You need them for the Harbinger of Hatred's Hoard, and if your route's messy, the whole farm starts to feel like work. The cleanest method I've used is still looping Urivar's Hoard in the Fields of Judgement over in Nahantu. Kill Urivar fast, spend 12 Judicator's Masks, crack the chest, and you'll usually pull 1 to 3 hearts. It's simple, repeatable, and honestly way better than wandering around hoping for lucky drops. If you're the sort of player who likes to keep runs efficient, some people even pair their grind planning with trading resources through sites like U4GM, especially when they want to save time gearing up for faster clears.

Why Urivar still feels the best

The big reason this farm works is speed. That's really it. On a mobile build, especially Spiritborn or a Rogue with strong movement tools, the whole run can be over before it starts to drag. You go in, burst the boss, open the hoard, reset, and do it again. There's no long setup and no awkward downtime. You very quickly notice that movement matters more than squeezing out one more damage multiplier. If you're already killing Urivar comfortably, shaving seconds off your route does more for your hourly returns than chasing a tiny DPS gain ever will.

Where the masks actually come from

Of course, none of that matters if your supply of Judicator's Masks dries up. That's the part a lot of players underestimate. The most reliable way to refill is still high-tier Nightmare Dungeons and solid Pit pushes, somewhere above Tier 100 if your build can handle it. If you're bored of that, Kurast's Undercity is a nice switch. Throw in a Tribute of Titans, add Seething Opals, and the place gets busy fast. Malignant enemies and Grotesque Debtors can drop hearts directly, which makes the run feel a bit more rewarding on the spot. In my experience, that route isn't always as steady as Urivar, but it's less repetitive, and in a group it gets even better because you can split the tribute cost and keep the pace up.

A farming loop that doesn't burn you out

The players I know who stick with this farm for weeks don't just slam one activity for hours. They rotate. Maybe 45 minutes in the Pit, then they cash out at Urivar for another stretch. It sounds basic, but it works. You keep masks coming in, your materials don't bottom out, and your gold stays healthier too. Helltides are worth sliding into that routine as well, especially in Kehjistan or Nahantu. They're more relaxed, less boxed-in, and good for stocking extra masks through event farming and chest openings. If Andariel's Claw is available, that chest should be your first stop, because the mask return is hard to beat for the time spent.

Small things that make a big difference

A few details really change how smooth this grind feels. First, make sure you're on Torment, because hearts won't drop if you aren't. Second, don't obsess over perfect damage if your build already kills comfortably; movement speed, cleaner routes, and fast resets usually pay off more. Third, bank your masks and open several hoards in one session instead of stopping every time you get enough for a single run. And if you're short on currency while putting the whole setup together, keeping some Diablo 4 Gold ready for crafting, rerolls, and general upkeep can make the farm feel a lot less annoying while you stack hearts at a steady pace.
